This form will be used to customize your opt-in offering and set up your MailChimp account, create a list, create your optin, create the response email with the double opt in and then the pdf and put it on your DSD coach site. Be sure that you check and double check your responses as these will be used to customize and any changes will incur more charges to you.

Although we will often complete the work quicker, please allow 7-14 working days for completion.

You will receive a copy of the customized pdf at the same time it is forwarded to be inserted into your Mailchimp Account, so that will indicate that we are working on it and about where we are in the process. IMPORTANT: We will not be contacting you. Everything we need from you is in this form.
